Johnson M.S. HVAC Make-up Air Unit Replacement - Close Out Change Order #1
On 1/21/2014 the School Board approved Mid-Sate Mechanical, Inc. to provide MEP Design Build Services under their Continuing Contract for the HVAC make-up air unit replacement at Johnson Middle School for a Guaranteed Maximum Price (GMP) in the amount of $300,205.26.
The work has been completed. Reconciliation requested by the District has resulted in a credit for unspent contingency, sales tax savings and Owner Direct Purchases (ODP) in the amount of $116,871.98. The ODP portion of the credit in the amount of $95,625.78 is an administrative change only because the material and equipment was purchased directly by the District for installation by the construction manager. The remaining $21,246.20 will be returned to the Capital Fund to be used on future projects.
Because the budgeted cost of construction is over $300,000.00, the Florida Department of Education (FDOE) requires the Superintendent to sign the Certificate of Final Inspection (OEF-209) following certification by the Engineer/Architect of Record and the Building Official.
Approve Change Order #1, Project Closeout and Release of Final Retainage to Mid-Sate Mechanical, Inc. for the HVAC Make-up Unit Replacement at Johnson Middle School under their Continuing Contract for MEP Design Build Services. Change Order #1 will decrease their Guaranteed Maximum Price by $116,871.98 to $183,333.28 with no change in contract time.
Authorize the Superintendent to sign FDOE OEF Form 209 - Certificate of Final Inspection for the project.
